There have been a few questions regarding the Personal Health Assessment that I would like to share and clarify for all employees wishing to participate in the Healthy Habits Challenge.
1. What information do I need to conduct a Personal Health Assessment?
You will need to know your height, weight , waist girth, blood pressure, glucose (blood sugar), cholesterol and triglyceride values. These results should not be older than a year to get an accurate assessment.
You may contact your health provider for these results or use the results given to you at the Know Your Numbers Health Fair that we conducted in November 2009. The results of the screening were placed in your Know Your Numbers Journal. You may use these results to help you fill out your assessment.
If you still do not have all of the information you need, complete the assessment as best as you can. You can return later to add or update information.
2. Can I still earn my 300 points if I do not complete my Personal Health Assessment this week?
YES!! The emphasis is to complete it this week but as long as you have it done by the end of the program on April 30, 2010, you can claim your 300 points.
3. Do I have to maintain all of the healthy habits throughout the program?
The intent of the program is to develop certain health activities into daily healthy habits. However, you will only earn points for the health activity specified for that week except for the Personal Health Assessment points that can be earned up until the end of the program. For example, during week two, you will earn points each day you engage in physical activity for at least 30 minutes. During the remaining weeks, we would like for you to continue exercising but you cannot further claim points for physical activity.
Remember, that the attached GBS tracking sheets are only intended to help you track your daily activities. You do not have to use them and the file name may not correspond to the actual week number in our program. For example, the Food Journal tracking Form has a # 2 in the file name but we are focusing on doing a food journal during week #3.
